CARDIAC REHABILITATION


PROGRAM
As a participant in Phase III cardiac rehab, you should see no difference in the day-to-day operations of the program. Angela Schaufert and Becky Davis will continue to provide you with excellent medical supervision of your exercise program. The program will operate Monday, Wednesday, and Friday at the Dearborn County Hospital from 8am to 12pm beginning in June, 2008.

LOCATION
The space for Phase III Cardiac Rehab is located in the lower level of the hospital's new addition and will be dedicated to our program during our regular program hours. Our new location will be approximately 1200 square feet with a private restroom. Parking will be available in the parking garage at no cost. Currently, construction on this space is just beginning, so we will share photos and updates as they become available.

FEES
The fee structure for the program will change only slightly and this letter contains your customized fee based on your participation in Silver Sneakers, Scholarship or Spousal Discounts. Click here to print off a fees and payment form (.pdf).

  • There will no longer be Cardiac Fees and YMCA Membership fees. The flat fee for Phase III rehabilitation services will be $50.00 per participant per month.
  • You will be able to elect only one type of discount, if applicable. For example, if you have Silver Sneakers, you will not be able to take the Spousal Discount as well.
  • Spousal Discount: A discount of 25% will be extended to spouses who currently are participating in the program or would like to in the future.
  • Scholarships: Scholarships will continue to be available to all participants based on need. Scholarships application can be made at any time and only requires a completed application and a copy of your last tax return. Scholarships are reviewed annually.
  • Silver Sneakers: Silver Sneakers will be honored through this program. Silver Sneakers is a program provided by several health insurance companies including Anthem, Humana, and AARP. If you have one of these Medicare supplement providers, let us take a look at your insurance card to see if you qualify for a discount. Silver Sneakers participants will not only receive a discount on their rehabilitation fees, but they will be able to use the facility from 8am to 12pm on Tuesday and Thursday as well.
  • Program fees can be paid monthly via Electronic Funds Transfer (EFT) or cash.

IMPORTANT DATES

  • The Dearborn County YMA Cardiac Rehabilitation program's tentative first day at the Dearborn County Hospital will be June 2, 2008. This date is subject to change based on completion of the facility. As we move forward, we will keep you informed on any construction delays which may affect this final date.
  • Until the new facility is complete, Cardiac Rehabilitation will continue to meet at our current location at 404 Green Blvd., Aurora.
  • If you choose to pay your fees by having the funds electronically transferred, you will need to complete the enclosed EFT form by March 15, 2008.
  • For the month of March, you will only pay your current Cardiac fees (no YMCA membership fees). Beginning in April, you will pay the new Cardiac fee listed on the following page.
